Notice of Name Change - AliMed, Inc. to AliMed, LLC
To our Valued Customers:
We are writing to inform you of a recent change to our legal name and corporate form. Recently, AliMed, Inc. completed an internal reorganization and is now operating under the name AliMed, LLC.
This reorganization was undertaken for internal structural purposes only and does not affect the ongoing business relationship between our companies. AliMed, LLC is the same business with the same assets, liabilities, and operations that previously conducted business as AliMed, Inc. and please continue to accept payments to AliMed, Inc. as we transition our Customers to the new name.
Please note the following:
- Our federal Tax ID number remains the same: 04-2577770.
- Our business and operations have not changed as a result of the restructuring.
- Our mailing address, phone numbers, email addresses, and website remain the same.
For your records, please update our name in your systems from "AliMed, Inc." to "AliMed, LLC." Going forward:
- Please make checks and other payments to AliMed, LLC.
- Please address all correspondence and invoices to AliMed, LLC.
- No changes are required to our Tax ID number or your existing account set up with us.
- An updated IRS Form W-9 reflecting our new legal name is available upon request.
